St. Francis Home

St. Francis Home is a 146-bed long term care facility. All rooms are private rooms, with the exception of two double-occupancy rooms.  Our private rooms promote the privacy, dignity, and individuality of our residents.

St. Francis Home is licensed by the Ohio Department of Health and is Medicare and Medicaid certified to provide intermediate and skilled nursing care. The facility is located in the south end of Tiffin on the picturesque groundss of our sponsor, the Sisters of St. Francis.  The peaceful, wooded campus invites residents to enjoy nature at their doorstep.
 
Nursing Home

We strive to enhance the physical, emotional and spiritual well-being of our residents by recognizing their individuality and by providing them with opportunities to live a full and satisfying life.

Our caring and supportive staff takes a special interest in each resident’s well-being, assisting them when they need help while encouraging them to remain as independent as possible for as long as possible.

Our services and programs include:

  • Three nutritious meals & snacks

  • Daily housekeeping of residents’ rooms and all common areas

  • Daily laundry of personal washable garments

  • Hair care provided by a licensed barber and beauticians

  • Transportation

  • Emergency call system

  • Religious services daily in a large, centrally-located chapel

  • Spiritual support as residents and family desire

  • Life enrichment activities that include education, social, recreational and intergenerational programs

  • Physical, occupational, speech and respiratory therapies
Visits with family and friends are encouraged. Activity rooms are located throughout the facility for formal and informal gatherings.  Guests are welcome to dine with residents with prior arrangement. A fee is charged.

Skilled Nursing

St. Francis Home provides care to residents who require short-term stays as a result of medical or surgical conditions that require treatment for a limited period of time. These services may include restorative care and/or physical, occupational, speech or respiratory therapy.

Since Medicare certification was obtained in 1997, residents can return to St. Francis Home after a hospital stay to continue their treatment. This provides comfort and security in a familiar environment with familiar caregivers tending to their needs. All the rooms in St. Francis Home are dually (Medicare and Medicaid) certified to minimize the transfer of residents from one room to another, depending on the level of care required at the time.

Hospice Care

Hospice care is available to residents of St. Francis Home.  We contract with two hospice providers, Community Hospice Care and Heartland Hospice.  Residents have their choice of provider.

Respite Care

Respite care is available for those who need short-term care.  Individuals and families who care for relatives can avail themselves of respite care when they must travel, or when they need a break from their daily care schedule.
