What is St. Francis Senior Ministries’ Notice of Privacy Practices?
Residents have the right to adequate notice concerning the use and disclosure of their protected health information (PHI) on the first date of admission. And, new notices will be issued when St. Francis Senior Ministries’ privacy practices change.
The Notice of Privacy Practices:
- Contains residents’ rights and the facility’s legal duties.
- Are available to residents in print.
- Are displayed in the reception area of the facility.
What are Resident Privacy Rights?
St. Francis Senior Ministries permits residents to exercise their rights over their PHI. It is our job to make sure that residents can exercise their rights, including the following:
- Receive Notice of Privacy Practices at the time of admission.
- Request restricted use and disclosure, although St. Francis Home is not required to agree.
- Have PHI communicated to them by alternate means and at alternate locations to protect confidentiality.
- Inspect and amend PHI and obtain copies, with some exceptions.
- Request an accounting of disclosures for six years prior to the request, except for disclosures made for treatment, payment, healthcare operations or with prior authorization.
- Contact designated persons regarding any privacy concern or breach of privacy within St. Francis Home, Elizabeth Schaefer Apartments or St. Francis Villas.
